Our Mission

 Our mission is to provide direct in-home access to intermediate healthcare for those that are indigent and underserved. We aim to help bridge the gap in primary care to reduce the need for emergency room visits.

 A Community Paramedic is a licensed EMS professional with additional certifications and training to provide non-emergency, in-home care. Instead of responding to 911 calls, they focus on helping patients manage chronic conditions, recover after hospital discharge, and avoid unnecessary ER visits. They work under the guidance of medical providers and are part of a growing national effort to expand access to care outside traditional healthcare settings. 


 While 911 EMS responds to emergencies, we focus on follow-up care, wellness checks, and low-acuity issues. We work in partnership with EMS agencies to ease their workload and ensure patients still receive appropriate medical attention. 


 No. We are a non-transport medical service. Our licensed paramedics provide in-home care for patients with non-emergency needs — helping them avoid unnecessary ER visits or ambulance rides.  Using our Community Paramedics as Provider extenders, we are able to escalate for telehealth services if needed. 


 Our services are available to anyone, regardless of age, income, insurance status, or background. However, we specialize in treating patients with mild to moderate medical needs who are safe to be seen at home instead of the emergency room. If you’re unsure whether your condition qualifies, a quick referral or consultation can help us determine the right fit.  Community Thread Health is committed to delivering care where it’s needed most — no barriers, no judgment. 


In many cases, it is free! We partner with local hospitals and EMS agencies which allows us to keep out-of-pocket costs low or non existent for patients. There is a proper referral process that has to happen for this though. We do offer some sliding scale direct care services without a referral, but this will be at a cost to the patient depending on insurance coverage. 


We are available whenever we are needed! We have expert staff on shift 24/7, 365 days a year. 


No. We complement primary care providers (PCPs) by providing services and support in the home between visits. Our goal is to help patients follow their care plan and avoid unnecessary ER visits or hospital readmissions. We do strive to communicate with the PCPs in order to further the continuity of care for all. If you do not have a PCP, we will work with you to find a PCP that fits your needs regardless of insurance status. 


Not yet. We are in the works of partnering with the needed teams to be able to provide these services. This is something that our Directors are working diligently on and once we are able to, we will be more than happy to assistant with compassionate care for those needing Behavioral and Mental health services.
