Our team
Community Thread Health is led by a passionate team of licensed paramedics, nurse practitioners, and healthcare innovators committed to closing the gaps in access to care. With deep roots in the communities we serve, our team brings together clinical excellence, operational experience, and a shared mission to meet people where they are—literally. Our structure includes a fully CPOM-compliant PLLC, guided by a Texas-licensed physician who oversees protocols and clinical standards, and a Management Services Organization (MSO) that handles operations, logistics, and staff support. Together, we deliver high-quality, compassionate care to those most often left behind by the traditional healthcare system. At every level, our people are driven by one goal: keeping patients healthy, safe, and supported—at home and in the community.
